Usually 50+ years old Registered with MK GP's or Living in MK Council Boundary History of fall identified at risk of fall or fear of falling
- GP
- Any health and social care professional including Ambulance Services
- Self-refferal
- We accept Electronic Referrals (e.Rs) directly from SystmOne.
- We accept referrals by secured email & by Post.
- We accept self-Referrals by Phone
