B-Well Group Provides Integrated Falls Prevention Service In Milton Keynes

The service provides assessment, treatment, and advice to older people in a supportive environment. People can be assessed in their own homes/residences, and in cases where exercise classes are required, they are available free of charge to help reduce the risk of falling and improve mobility and balance. We provide all our services in the community; that is, we see our clients at their homes or residences only. Additionally, we provide exercise classes in the community, and clients are required to attend these locations.

We are a Multi-Disciplinary Team (MDT), a collaborative group of healthcare professionals from various disciplines like Nurses, Physiotherapists, Occupational therapist, Therapy assistant, Exercise instructors and care navigators working together to prevent, assess, and manage falls in older adults. Falls are a complex issue with many contributing factors, and the MDFT takes a holistic approach to reduce risk and improve outcomes for individuals. Our goal is to provide comprehensive, individualised care to promote safety, mobility, and quality of life for those at risk of falls.

Physiotherapists specialised in Falls Prevention: help in assessment, diagnosis and treatment plan related to individual's strength, balance, and mobility, all of which will help to reduce falls risk.
Occupational therapists specialised in Falls Prevention: help in assessing and advising different assistive devices and techniques to improve daily activities which will help to reduce the falls risk.
Nurse Specialist in Falls Prevention: help in checking body routine vital signs and some special tests for the early identification of people with falls risk.
Exercise instructors in Falls Prevention: help to deliver dedicated, progressive exercise programmes individually and as a group to reduce the falls risk.
Care Navigators in Falls Prevention: help to facilitate seamless communication between clinical team members, service users and their registered GP/practice.
